Solventum offers three medical plan options designed to support preventive care, everyday health needs, prescriptions, behavioral health and more complex care when you need it:

  • Choice Advantage Plan
  • Prime Advantage Plan
  • Basic PPO Plan

Each plan includes the same core medical and prescription drug benefits, plus access to additional medical programs and resources designed to support you and your loved ones when you need it most.

The plans vary in cost and in how you pay for care, giving you options to choose the coverage that best fits your health care needs, budget and family situation.

Plan

Cost structure

May be a good fit if…

Choice Advantage Plan

  • Lowest payroll contribution
  • Higher cost when receiving care

You want to keep more of your paycheck now and are comfortable paying more if you need care.

Prime Advantage Plan

  • Moderate payroll contribution
  • Moderate cost when receiving care

You want a balance between your monthly cost and what you pay if/when you receive services.

Basic PPO Plan

  • Highest payroll contribution
  • Lower cost when receiving care

You prefer to pay more each month in exchange for lower costs when you receive care.

Choosing a medical plan starts with understanding how the plans work and what your costs may be throughout the year. One cost to consider is your premium, or the amount you pay from your paycheck for medical coverage. Depending on the plan you choose, you may also pay out-of-pocket costs when you receive care, such as deductibles and coinsurance.

While all three plans provide comprehensive coverage, there are differences in:

  • What you pay from your paycheck for coverage
  • How prescription drug benefits work
  • Whether you can contribute to a tax-advantaged Health Savings Account (HSA)
  • Your deductible and out-of-pocket costs
  • In-network preventive care covered at 100%
  • No referrals required
  • Prescription drug coverage
  • Behavioral health coverage
  • Access to additional medical programs and resources
  • Protection through an annual out-of-pocket maximum
  • Travel benefits to help support access to care when travel is necessary for certain services
  • Payroll contributions
  • Deductibles
  • Cost-sharing structure
  • When prescription drug cost-sharing begins
  • Eligibility for a Health Savings Account (HSA)

1. The annual deductible and out-of-pocket maximum (OOP max) can be met through a combination of medical and prescription drug expenses.
2. The annual deductible is only met by medical expenses. The OOP max can be met through a combination of medical and prescription drug expenses.
3. There are separate deductibles and OOP maximums for in-network care and out-of-network care, which accrue separately.
4. The Choice Advantage and Basic PPO plans have an embedded deductible. This means the plan begins paying benefits for each enrolled family
member after they meet their own deductible, even if the family deductible isn’t met.
5. The Prime Advantage Plan has a non-embedded deductible which means, the entire family deductible must be met before the plan begins paying benefits for any enrolled family member. Family OOP max has an embedded per member OOP max of $9,200 for in-network services.
6. Percentage of allowed amount after deductible. The allowed amount reflects how much the plan will pay for the medical expense. If your provider charges more than the allowed amount, you may have to pay the difference (which does not count toward your annual deductible or OOP max).
7. Virtual health visits through Doctor on Demand, Virtuwell and Teladoc are not subject to your Solventum health plan’s deductible.
8. The annual Solventum HSA contribution amount is prorated for employees hired after January 1.

The chart below shows your share of the monthly cost for medical coverage before and after applicable premium discounts. Your actual cost may vary depending on your eligibility for well-being incentives, the tobacco-free discount or any applicable surcharges.

Remember, the amount deducted from your paycheck is only a portion of the total cost of coverage. Solventum pays the other portion, which is much larger. This investment by the company helps make comprehensive healthcare more affordable and accessible for you and your family.

The Choice Advantage and Basic PPO plans have embedded deductibles. This means one covered family member can meet their individual deductible and begin coinsurance before the full family deductible is met.

The Prime Advantage Plan uses a non-embedded deductible for family coverage. This means the full family deductible must be met before the plan begins paying for any covered family member.

Compare your options and find the plan that works best for you and your situation.

Understanding how medical plans share costs can help you choose the plan that’s right for you.

Each Solventum medical plan pays 100% for routine preventive care like annual screenings, with no out-of-pocket costs when you visit an in-network provider.

For most non-preventive care, you pay the full cost of covered services until you meet your deductible. After that, you share costs with the plan through coinsurance. If you reach your in-network annual out-of-pocket maximum, the plan pays 100% of eligible medical and prescription drug expenses for the rest of the year. Most other medical expenses follow this general progression:

You pay all costs for covered services until your deductible is met.

After meeting your deductible, you will share the costs of services through coinsurance.

Once the deductible and Out-of-Pocket (OOP) maximum have been met, the plan covers 100% of eligible in-network medical and prescription drug expenses for the rest of the year, leaving you with no additional costs.

Your medical plan includes access to a provider network that is determined by where you live. Depending on your home ZIP code, you may be enrolled in a nationwide network or a regional network. You do not select your network or medical plan administrator during enrollment.

Before enrolling, we encourage you to confirm that your preferred doctors, specialists, hospitals and healthcare facilities participate in your network. To search for providers, visit the Blue Cross Blue Shield provider directory or the HealthPartners provider directory.

Prescription drug coverage, administered by CVS Caremark, is automatically included with each Solventum medical plan. The medications covered are consistent across the plans, but how you pay for prescriptions depends on the medical plan you choose. Some medications may be covered before you meet your deductible, while others follow your plan’s standard cost-sharing rules.

Prescription drug lists (also known as formularies) are updated regularly to ensure covered medications are safe, effective and affordable. If you are impacted by formulary changes, you will be contacted directly by CVS Caremark.

Your prescription costs depend on the medical plan you choose and the type of medication you need. The medications covered are consistent across the Solventum medical plans, but when cost-sharing begins may differ by plan.

With the Choice Advantage and Prime Advantage plans, you generally pay the full cost of prescriptions until you meet your deductible, unless the medication is on the Preventive Drug List. With the Basic PPO Plan, prescription cost-sharing begins right away because prescription drugs are not subject to the medical deductible.

Some preventive medications are covered before you meet your deductible. These medications are included on the Preventive Drug List and are designed to help prevent or manage certain health conditions. Some preventive drugs required under the Affordable Care Act may be covered at 100%. For a complete list, visit caremark.com or call CVS Caremark at 833-992-2784.

Certain specialty medications may qualify for copay assistance from drug manufacturers. CVS Caremark can help eligible members enroll in these programs. Copay assistance may reduce what you pay for the medication, but it does not count toward your plan deductible or out-of-pocket maximum.

Prescription drug lists are updated regularly. To find out whether a medication is covered, how much it may cost or whether there are lower-cost alternatives, contact CVS Caremark or visit caremark.com. If a coverage change affects you, CVS Caremark will contact you directly.

Term

What it means

Generic drug

A medication that has the same active ingredient as a brand-name drug and is equivalent in dosage, safety, strength, quality, performance and intended use.

Brand-name drug

A medication marketed under a trademark-protected name.

Preferred drug

A generic or brand-name medication that generally costs less under the plan.

Non-preferred drug

A medication that generally costs more under the plan.

Maintenance medication

A medication you take regularly to manage an ongoing health condition.

Prescription drug list

A list of medications covered by the plan. This may also be called a formulary or preferred drug list.

The amount you pay for medical coverage may be lower or higher depending on your participation in certain Solventum well-being programs and your family situation.

Solventum rewards healthy habits through the Healthy Living Incentive program. Employees and covered spouses or domestic partners who complete the required well-being activities may qualify for a discount on their medical plan premiums.

Employees who are tobacco-free may qualify for a monthly premium discount. An additional discount may also be available if covered dependents are tobacco-free. Employees who use tobacco may still qualify for the discount by completing an approved tobacco cessation program.

If your spouse or domestic partner has access to medical coverage through their own employer but enrolls in a Solventum medical plan as their primary coverage, an additional surcharge of $100 per month may apply. This includes a spouse or domestic partner who is a current Solventum employee or a Solventum retiree eligible for the retiree medical HRA plan. If your spouse or domestic partner is unemployed, is not offered employer-provided coverage or is covered under a Solventum medical plan as secondary, no surcharge will apply.
